Increasing NMS in Anticoagulation across Wessex - new 'Jack' cards to be issued to eligible patients

The attached card is planned to be issued to patients through secondary care outpatient anticoagulation clinics. Patients will receive initial counselling and then the card will be given to them.  

The card does not specifically ask for an NMS review, but does guide, direct and encourage the patient to ask to speak to their pharmacist about their medicines - one of which will be a new anticoagulant, and hence eligible for an NMS. I hope this encourages more patients to visit our pharmacies, and more pharmacists to be able to provide greater care to these patients, whilst benefiting all contractors with increased NMS service provision & revenue.
